The commissioner said he has found the explanation given unacceptable and it reflects a breakdown in standard operation procedures, which eventually resulted in the tragic deaths of two persons.
The Bureau of Special Investigations (BSI) and the Independent Commission of Investigations (INDECOM) are investigating the matter.
Ellington has asked that this investigation be quick and thorough.
He said he has also asked that the advice of the Director of Public Prosecutions be sought on the appropriate course of action to take.
